How to apply for Membership


  1. Complete the form below. We recommend you print out a copy for your records before submitting the application.
  2. After carefully reviewing your entries and selections, click on the "Submit Application" button.
  3. Mail the required dues for membership to the address indicated below. Please annotate your check with "GMA Membership Dues" and your clearly printed name.
  4. When association officers receive your dues and approve your application, the information you enter on this form will be posted on the membership directory page.
  5. If you believe the association should know other information about you or your practice please send an email to info@gamediators.org

If you need to make changes to information you submitted, please send an email to info@gamediators.org with the appropriate information.

Dues: Association membership dues are $50.00* per year (General Member-must be registered neutral with the GODR)
$25.00 * (Associate Member-not state-registered or not a mediator)
$25.00 * (Student member)

*Membership privileges will begin upon receipt and will extend through December 31, 2012.
